Please note that this online scheduling form is for injection appointments and skin care consultations ONLY, and you can NOT schedule skin care treatments online.
Consult fees go towards the procedure. Consult fees are not refundable unless canceled 24 hours before appointment, however they remain in the patients account for 2 years. Surgical consult fees are not transferable to Medspa unless the patient is not a candidate for surgery.
